Poches de stomie convexes : une solution pour vos patients ?

Des complications liées à la stomie peuvent survenir dans le mois suivant l’intervention, mais également à un stade ultérieur. Certains des facteurs contributifs peuvent être la localisation de la stomie, ses caractéristiques, la formation d’une fistule ou un débit élevé.. Lorsque l’un de ces obstacles survient, les effluents peuvent irriter la peau péristomiale et même s'infiltrer sous le support.2 Par conséquent, l’adhérence du dispositif peut être compromise et la zone environnante peut également être irritée.

Les poches convexes peuvent aider à éviter les fuites

Un système de poche de stomie convexe peut éventuellement permettre à l’appareillage de mieux adhérer à la peau péristomiale. Ceci afin d'éviter les fuites qui constituent une préoccupation fréquente chez de nombreux stomisés.2 Les poches de stomie convexes sont conçues pour : 1,2

 

  • Appliquer une pression suffisante autour de la stomie pour lisser les contours péristomiaux, en particulier dans les plis cutanés, afin de vous assurer que le support forme une étanchéité avec la peau.
  • Exercer la pression adéquate sur la peau péristomiale pour permettre à la stomie de ressortir davantage à l’intérieur de la poche.

Quand devez-vous suggérer un appareillage de stomie convexe ?

Chaque stomie est différente et souvent, la convexité peut être une grande aide. Pour choisir le bon appareillage, celui-ci doit être adapté aux conditions anatomiques et dynamiques de l’abdomen du patient ainsi qu’aux caractéristiques de sa stomie. Par conséquent, les systèmes de poches de stomie convexes diffèrent entre autres en termes d’angle, de forme et de profondeur de la convexité.

En fin de compte, c’est votre expérience clinique qui détermine si vous recommandez un système convexe à votre patient et quelles propriétés il doit posséder.

Les appareils convexes peuvent être appropriés pour1

  • Stomie affleurante : une stomie doit mesurer 2,5 cm de long afin que l’étanchéité autour de la base de la stomie puisse fonctionner correctement. Toutefois, si une stomie se situe au niveau de la peau abdominale, elle est considérée comme affleurante.
  • Stomie rétractée : une stomie rétractée est située sous le niveau de la peau. Cela peut être dû à une difficulté technique pendant l’intervention chirurgicale ou à une prise de poids postopératoire.
  • Stomie télescopique : en particulier la nuit, lorsque les patients sont allongés, la stomie télescopique peut glisser vers ou sous le niveau de la peau.
  • Stomie mal placée : si une stomie s’est formée au cours d’une intervention chirurgicale d’urgence alors que le patient était en décubitus dorsal ou avait un abdomen distendu, le résultat peut ne pas être optimal.
  • Stomie à haut débit : lorsque les effluents de la stomie dépassent 750 ml par jour, la probabilité de fuite augmente. Il en va de même si la production est de nature plutôt liquide.
  • Plis de la peau péristomiale : la peau peut former des plis semblables à des canaux qui provoquent des fuites.
  • Abdomen mou : un abdomen dont la tonicité musculaire est faible fournit peu de soutien à la stomie.

Convexité légèrement inclinée

La convexité en pente douce est une caractéristique de tous les produits de 4 à 6 mm. Elle a été développée pour assurer une meilleure répartition de la pression, et appliquer une pression suffisante autour de la stomie afin de mettre ses contours en avant. 

Intensifier l’effet de convexité avec une ceinture de stomie

Pour créer un sentiment de sécurité, de nombreux utilisateurs recommandent l’utilisation d’une ceinture de stomie pour intensifier l’effet de convexité. Trois positions de fixation différentes* permettent d’adapter la ceinture à la position préférée par le patient, de la poche sur l'abdomen.
